This page contains PDF versions of all of the forms that are needed to
register for Chili Lions Youth Football and Cheerleading. You will need to have

1. Pop Warner Registration

This is a two sided form that is used by
Western New York Pop Warner to register
participants. This must be printed on one sheet
of paper.
2. Parent/Participant

This is an agreement between Chili Lions Youth
Football and the Participant and their Parent. 3.

This provide medical information and permission
to provide medical assistance to a participant. 4.

This for must be given to your doctor to approve
your child's participation. 5.
Additional items you will need to be properly registered.
a. Registration fee of $120 ($100 if you register before March 5, 2006).
b. Small photograph of you child attached to the Pop Warner form.
c. A copy of your child's birth certificate or other proof of age
d. Participation in the mandatory fundraiser (prepayment for candy or candles -
$104)
You may buyout of the fundraiser by paying and additional
$54.
e. A copy of your medical insurance card.
f. If registering for Football, your child needs to be present to be weighed in. |
